FOR THE PATIENT. FOR THE FAMILY.

Support at home.
Throughout a difficult chapter.

When a cancer care plan includes needs at home, families deserve a clear place to start. Ask IPH about nursing, therapy and coordination with your oncology team.

Professional support at home
Professional care. A human touch.

One request. A team to guide you.

Review of home health needs with the treating oncology team

Questions about nursing, rehabilitation and ordered support

03

A straightforward request for patients, families and referring offices

IPH’s supplied Face to Face form lists Cancer among its programs. This is support alongside the treating team, not a promise of an oncologist visiting at home, chemotherapy, radiation, hospice or a cancer cure. IPH confirms the actual ordered services, eligibility and availability.

Your first questions.

Does this replace my oncologist?

No. Your oncology team continues to direct cancer treatment. IPH reviews which ordered home health services may support you at home.

Can a family member start the request?

Yes. Choose the family route and provide your own contact details. IPH will confirm authorization and any patient information needed.

Is cancer care the same as hospice?

No. Home health, palliative care and hospice are different services. IPH must confirm which services it can provide; this page does not enroll you in hospice.

Can an oncology office refer a patient to IPH home health?

Yes. The office can submit a professional intake request with its contact details and available Face to Face information. IPH reviews the ordered home health services, documentation, location and coverage.

Can IPH confirm chemotherapy at home through this form?

No. This intake does not confirm chemotherapy, radiation or specialized cancer treatment at home. Ask IPH and the treating oncology team which specific ordered services are available.

What can families ask about when requesting home health during cancer care?

Families can ask about ordered nursing, rehabilitation, home health aide support and medical social work listed by IPH. The team confirms what fits the care plan and is available for the patient.

Does having cancer automatically qualify someone for home health?

No. A diagnosis by itself does not establish coverage or admission. IPH reviews skilled-care needs, orders, the applicable plan and eligibility requirements, location and service availability.

What if I am unsure which home health service a person with cancer needs?

Start with the family or patient request and choose the cancer support area. You do not need to select a treatment yourself. IPH can discuss the next steps with you and the treating team.
